- RETIERED 11 August 2026 under D-028 against EVIDENCE_PROTOCOL's six-level source hierarchy. The previous value was an ad hoc quality score on an undocumented scale. Tier now records SOURCE TYPE only; how far this project has checked the document is the Status column. Previous value: tier 2. Opened 10 August 2026 by rendering. It is a two-page extract, not the plan. Page 1 carries the Introduction, describing the Humanitarian Programme Cycle, the roles of the Humanitarian Country Team, the Humanitarian Coordinator and OCHA, and the framework of agreements between the Syrian State and UN agencies. Page 2 carries only the date, 2 April 2026. It does NOT carry the population or needs figures, so 25.6 million, 15.6 million and 16.5 million are not verified by this document and remain unverified against any OCHA publication. One of the three is corroborated elsewhere: UNHCR's May 2026 operational update at SRC-024 gives 15.6 million requiring humanitarian assistance, so that figure now rests on a primary document even though the OCHA plan itself is unopened. The full plan is still to be obtained
- THE 10 AUGUST 2026 ATTEMPT, RECORDED HERE 11 SEPTEMBER 2026 on the batch A diff review's register note, because Chapter 1 line 235 compresses it and this row had never carried it. The office's own site, the OCHA family of hosts, returned HTTP 403 Forbidden to WebFetch on 10 August 2026 on every host tried, per سجلّ عمل داخلي line 220; an ordinary client had been served the day before. SO THE OUTCOME WAS A REFUSAL, NOT UNREACHABILITY, and the prose says 'would not open', VOICE_STYLE's rendering of a 403, and may not say 'could not be reached'.
